Consultation
Your initial appointment will consist of a consultation determining your diagnosis and explaining treatment options. Occasionally, some limited procedures may be able to offered the same day as the consultation. A complex medical history or treatment plan will require a detailed evaluation and further appointments to provide treatment on another occasion.
Please assist us by providing the following information at the time of your consultation:
- Your referral letter and any x-rays if applicable.
- You will be asked to complete a full medical history form prior to seeing Dr. Bowler. This may be done online via this website.
- If you have medical or dental insurance please bring the necessary information.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.). If you are on any form of medication we need to know this as well, especially heart medications, Aspirin and anticoagulant therapy.
All vitamin and herbal products/medicines, such as vitamin C tablets, garlic preparations, Echinacea preparations, antioxidant tablets and vitamin E tablets can predispose to bleeding and should be avoided for 10 days before your operation.
X-Rays (Radiographs)
If your dentist or medical practitioner has taken or arranged x-rays, you may request they forward them to our office. If there is not enough time, please pick them up and bring them to the office. If additional films are necessary, some of these can be taken at our facility.
